Please click on the link above to download the appropriate nutritional analysis form. This form allows us to personalize your weight loss program prior to your your arrival. Once we receive your form, we will arrange a 15 to 20 minute phone conference to design your program.
Credit card authorization form
Please download, complete and sign the credit card authorization form to reserve your time with us. Please fax the signed form to (310) 576-7850. We require a deposit of half the cost of the first week to reserve your room. We have a 24 hour cancellation policy with a 100 percent refund. If cancellations are made with less than 24 hours notice we reserve the right to deduct the cost of one night from your refund.
Medical release
We prefer to work with your doctor as most of our clients begin to reduce their medications within the first week. We do not advocate the reduction of medications without doctor's approval.
